Student Health Services

University of Kansas Student Health Services requires several forms be read or filled out and sent to camp with the participant.

Summer Camp Letter
Summer Camp Flier and Confidential Record
Consent for Use or Disclosure
Treatment Agreement
Release of Liability

Your student needs to have their Confidential Record (second page of the Summer Camp Flier and Confidential Record pdf), Consent for Use or Disclosure, Treatment Agreement, Release of Liability and a copy of their insurance card (if available) upon the arrival at camp.

Where to Arrive

KJI students should arrive at registration between 1:30 and 3:30 p.m. on Sunday, June 23 at Oliver Hall.
